Carpal tunnel syndrome (CTS) has a complex pathophysiology with multiple predisposing factors, including wrist osteoarthrosis (OA), which can alter the contour and available space with the carpal tunnel. In this video, I present a pattern of wrist OA that narrows the carpal tunnel inlet and may predispose to CTS.
This case highlights the value of beginning the sonographic evaluation of the carpal tunnel with a limited dorsal wrist assessment and demonstrates how wrist osteoarthrosis, particularly in a scapholunate advanced collapse pattern, may contribute to the pathogenesis of CTS.
