In this Case of the Month, I highlight an acute ankle injury that resulted in persistent anterolateral ankle pain. Although injury to the extensor retinaculum is uncommon, it may go unrecognized and serve as an overlooked source of chronic symptoms. This case reviews the anatomy and sonoanatomy of the lateral fibers of the inferior extensor retinaculum, or frondiform ligament, and demonstrates how injury to this structure can be recognized with ultrasound.
